Our Process.

Paperwork:

When you arrive at our office, our administrative staff will ask for a prescription from your physician (one may have already been faxed to us beforehand) and your current/active insurance card(s). If you are new with us they will also have some intake forms to be filled out. If you are a previous patient please notify staff if things like your address, phone number, email or insurance has changed since last time we saw you. We appreciate it when patients arrive 5-10 minutes before scheduled appointments to allow time to take care of paperwork and any other items the administrative staff need to address.

Insurance:

Westlake Orthotics and Prosthetics accepts all insurance, but we cannot guarantee coverage or benefits until we have received proper information. Please make sure to bring any and all current/active insurance cards, workman's compensation or automotive claim information to your appointment. This will insure we received prior authorization when necessary.

Financial Policy:

Westlake Orthotics and Prosthetics bills your insurance directly. We send out statements when we received an explanation of benefits back from your insurance.

If a service is not covered by your insurance - We ask that custom items 50% is paid before we begin fabrication of the custom item(s) and 50% due at time of delivery, unless other payment arrangements are made prior to items being fabricated and delivered.

If you currently do not have insurance - Payment will be due in full before the delivery date. If item is custom made we ask that 50% be paid before we begin fabrication and 50% at time of delivery. Unless other payment arrangements have been made.

Cost:

Our fees for each device cover the provision of the item(s), evaluation, fitting, follow ups and adjustment appointments that pertain to that device(s) within the warranty period. Any major adjustments may be subject to additional costs but our staff will address that with you beforehand.

Warranty:

Our warranty is 90 days from delivery date and some manufacturer parts may have a shorter or longer warranty period. If adjustments or repairs are made outside of the warranty period we do bill for labor.
