What information / paperwork do I need to bring to my first appointment?

Please bring all insurance cards and a prescription if it was provided to you by your referring DR.

How long will my initial evaluation be?

Most orthotic appointments are 30 mins but we schedule for an hour. This ensures we can do a thorough exam and answer any questions you may have.

Am I / my insurance charged per visit?

You / your insurance company will NOT be billed per visit. We will only bill you / your insurance IF we provide a product to you. That charge will cover any additional appointments required ( example, follow up or adjustment appointments).

What Insurance do you accept?

We are in-network providers for many insurance carriers. Each insurance policy is different and coverage for orthotic and prosthetic services can vary depending on the device. It is important that you call your insurance company prior to your evaluation appointment to ensure Rebound Orthotics & Prosthetics, Inc. is an in-network provider. There should be a toll-free number on the back of your insurance card.
